Applications postmarked or received after May 20, 2008 will not be accepted
Your application cannot be processed unless this form is completely filled out, the vendor fee is sent with a copy of your proof of liability insurance coverage of $1,000,000*.
*If you do not have liability insurance coverage as stated, you can purchase it as an endorsement under the Tama-Toledo Chamber for an additional $80 along with your vendor’s fee.

**Note: If you do not request electricity no provisions will be made or allowed. All items are limited to 120 volt regular current, unless otherwise noted on application. Please understand that we may have to place a restriction on how much power is available upon receiving the total loads. Thank you for your cooperation - Electrical Chairman.
